Diagnosis of Erectile Dysfunction

Erectile dysfunction can be diagnosed to choose an appropriate erectile dysfunction treatment through the following methods


Erectile dysfunction can be diagnosed to select an appropriate treatment through the following methods: medical history, clinical examination, laboratory tests, penile Doppler ultrasound, measuring nocturnal erections, dynamic cavernosal imaging, penile vibrometer, and self-examination.

Medical history:

The doctor may be able to discover the cause by looking at the patient's medical history. The doctor will ask about:
  • smoking 
  • history of diabetes
  • History of high blood pressure
  • History of atherosclerosis
  • What medications the patient is using and whether they drink alcohol
  • Previous surgeries (for example, due to an enlarged prostate, surgery for prostate and anal cancer, or vascular disorders)
  • History of injuries (e.g., pelvic bone fracture or back injury)
  • Symptoms of vascular disorders (such as pelvic pain while walking or exposure to cold, numbness, or bluish feet)
  • Symptoms of neurological disorders (such as numbness, tingling, weakness, incontinence, or falling)
  • Symptoms of hormonal disorders (such as decreased sexual desire, increased breast size, decreased sense of taste, decreased body hair, trembling, changes in weight or appetite, difficulty tolerating heat or cold)
  • Symptoms of mental disorders, especially depression
  • Satisfaction with sexual relations 
  • Disruptions of sexual function (such as vaginitis or depression) in the sexual partner.

Clinical examination:

The clinical examination focuses on the genital area and the prostate. The doctor also looks for signs of hormonal, neurological, or vascular disorders and performs a penile and rectal examination.

The doctor will do a neurological examination to diagnose any abnormality in the brain that may affect a man's ability to have an erection.

Laboratory Test:

The doctor may order some important laboratory tests in diagnosing erectile dysfunction, such as the following:

  • Urinalysis detects infections of the urinary and reproductive systems of men, which may affect their ability to have an erection, and also helps to detect the presence of kidney problems or diabetes.
  • Blood glucose measurements are used to diagnose diabetes through a fasting blood glucose test or a cumulative glucose test.
  • Lipid analysis shows if there is high cholesterol or triglycerides in the blood, as well as any signs of heart disease or atherosclerosis.
  • Thyroid analysis, as an imbalance in any of the thyroid hormones, may affect a man's ability to have an erection.
  • Kidney function analysis, especially measuring creatinine in the blood, as well as liver function analysis. Kidney and liver disease may affect a man’s ability to maintain an erection.
  • Measuring the level of testosterone in the blood is important because testosterone deficiency is a major cause of erectile dysfunction.

Penile Doppler Ultrasound

The purpose of this test is to check the integrity of the vessels and arteries that supply the penis with the blood and oxygen needed for an erection properly during intimacy. This examination is considered one of the best tests required to detect erectile dysfunction and to ensure the integrity of the blood supply to the penis.

Occasionally a doctor can inject a drug into the penis to induce an erection, then use ultrasonography to measure the blood flow in the arteries and veins of the penis. In rare cases, doctors can recommend a home monitor that detects and records erections during sleep.

Measuring Nocturnal Erections

The patient's nocturnal erection is measured by using a specific device installed around the patient's penis that helps measure the rate of nocturnal erection of the penis, as it is normal for the penis to erect during sleep.

This examination is considered one of the most important analyses required to find out the cause of erectile dysfunction because, using this test, the doctor can determine if the cause is psychological or organic.

For example, an erection occurs upon awakening or during the night, which usually implies that there is not a physical cause for erectile dysfunction; rather, the cause is often psychological, as the physical injury disrupts the erection at all times. 

Other factors that suggest psychological causes include: In otherwise healthy young men, the condition develops suddenly, symptoms occur only in certain cases, and erectile dysfunction resolves without any treatment.

Dynamic Cavernosal Imaging

The dynamic cavernosal infusion examination, as it is called, helps in detecting the problem of venous leakage and malformation of the penis, which may cause erectile dysfunction as well as decreased sexual arousal.

Penile Vibrometer

A penile vibrometer is a device placed on both sides of the penis that emits electromagnetic vibrations that help examine the health of the nerves in the penis and measure its sensitivity.

Self-examination

The following warning signs can help patients to do a self-examination and know when they need to visit a doctor:
 
  • Absence of an erection during the night or upon waking in the morning
  • Sensation of numbness or tingling in the buttocks, genital area, or the area around it
  • Painful cramps in the muscles of the legs that occur during physical activity and temporarily subside at rest (claudication).
